Job Title: Adult Safeguarding Triage Social Worker (Locum)

Location: Oxfordshire

Rate: £35/hr - £38/hr

We are seeking a dynamic and dedicated Adult Safeguarding Triage Social Worker to join our team on a locum basis in Oxfordshire. As a pivotal member of our safeguarding service, you will play a crucial role in upholding the welfare of vulnerable adults in our community.

Responsibilities:

• Work within the requirements of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and any other relevant legislation, guidance, and codes of practice.
• Provide personalized information and advice to individuals, families, and their carers, utilizing strength-based approaches to explore all available options for support, including informal networks and community resources.
• Undertake timely and proportionate assessments, reviews, and professional interventions for individuals, families, and carers.
• Ensure that support plans accurately reflect the needs and outcomes of individuals, regardless of eligibility determination.
• Identify and coordinate necessary arrangements for services aimed at improving and maintaining independence, including reablement services and equipment provision.
• Conduct telephone enquiries, face-to-face visits in the community, facilitate clinics, and effectively manage and prioritize a caseload.
• Participate in duty, including providing telephone and emergency advice as required.

Requirements:

• 2+ years of post qualified experience
• Social Work England registration
• Strong assessment, communication, and interpersonal skills
• Ability to work autonomously and collaboratively within a multidisciplinary team
• Commitment to promoting the well-being and rights of vulnerable adults
