Adult Social Worker – Transitions Team

Pay Rate: £28.20 per hour
Hours: 37 per week

About the Role

Staffordshire County Council is seeking an experienced Adult Social Worker to join its Transitions Team.
You will support young people transitioning from Children’s Services into Adult Services, ensuring they receive the appropriate care, support, and guidance to promote independence and positive long-term outcomes.

This role involves close partnership working with families, education providers, health professionals, and multi-agency teams to deliver person-centred transition planning.


Key Responsibilities

  • Complete Care Act assessments and transition assessments.
  • Support young people moving from Children’s Services into Adult Services.
  • Develop person-centred care and support plans.
  • Work collaboratively with education, health, and community services.
  • Promote independence, wellbeing, and positive life outcomes.
  • Manage safeguarding concerns where required.
  • Maintain accurate and timely case recordings and reports.
