We are seeking a dedicated and experienced Adult Social Worker to join our Hospital Discharge Mental Health Team in North London. This is an exciting opportunity to make a real difference in the lives of individuals with mental health needs, facilitating their safe and effective discharge from hospital settings.

Key Responsibilities:

• Conduct comprehensive assessments of patients' needs and risks to ensure safe discharge planning.
• Develop, implement, and monitor care plans in collaboration with healthcare professionals, patients, and their families.
• Provide expert advice and support to multidisciplinary teams on mental health legislation and best practices.
• Work within statutory guidelines and policies, ensuring compliance with relevant legislation.
• Liaise with community services, healthcare providers, and other agencies to arrange appropriate follow-up care and support.
• Maintain accurate and up-to-date case records and documentation.

Requirements:

• Qualified and registered as a Social Worker with Social Work England.
• Proven experience in mental health social work, preferably within a hospital discharge setting.
• Strong understanding of mental health legislation and safeguarding procedures.
• Excellent assessment, planning, and intervention skills.
• Ability to work effectively within a multidisciplinary team and under pressure.
• Strong communication and interpersonal skills.
• Commitment to continuous professional development.
