Our Hospital Discharge Team plays a critical role in ensuring smooth transitions from hospital to home, and we are looking for a passionate and qualified Social Worker to join us in making a real difference.

Role Overview:

As a Social Worker in our Hospital Discharge Team, you will work alongside our four integrated Health and Social Services localities and our Hospital Social Work Team. Your primary responsibility will be to undertake targeted reviews of care packages, focusing on improving care outcomes and contributing to significant cost savings. This includes evaluating and adjusting care plans for individuals with Physical Disabilities and Learning Disabilities, early review of hospital discharges, and managing domestic care calls (cleaning, shopping, and housework).

Key Responsibilities:

  • Conduct comprehensive reviews of care packages, including double-handed care calls.
  • Perform early reviews of hospital discharges to ensure seamless transition and appropriate care.
  • Assess and coordinate domestic care calls for individuals with Physical Disabilities and Learning Disabilities.
  • Collaborate with multidisciplinary teams within the integrated localities to ensure cohesive care delivery.
  • Develop and implement strategies to improve care outcomes and achieve cost efficiency.
  • Maintain accurate and detailed records of assessments, care plans, and reviews.
  • Provide support and guidance to individuals and their families during the discharge process.

Requirements:

  • Qualified Social Worker with registered with Social Work England
  • Proven experience in adult social care, preferably within a hospital discharge or integrated care setting.
  • Strong understanding of care package assessments and reviews for individuals with Physical and Learning Disabilities.
  • Excellent communication and interpersonal skills to work effectively with clients, families, and multidisciplinary teams.
  • Ability to work independently and manage a diverse caseload.
  • Commitment to improving care outcomes and achieving financial savings for the local authority.
