Are you a dedicated and compassionate Adult Social Worker looking to make a difference in the community? Join our Locality Team in South East London and contribute to enhancing the lives of adults in need of support and care.

Key Responsibilities:

• Conducting assessments of need, care planning, and reviewing cases to ensure the well-being of adults in the community.
• Collaborating with multi-disciplinary teams, service users, and their families to provide tailored care and support.
• Managing a varied caseload, including complex cases, while maintaining detailed and accurate records.
• Implementing and monitoring care plans, ensuring they meet statutory requirements and best practice guidelines.
• Providing crisis intervention and safeguarding vulnerable adults, ensuring their safety and welfare.
• Advocating for service users and promoting their rights and independence.

Requirements:

• A degree in Social Work and current registration with Social Work England.
• Proven experience in adult social work, preferably within a locality team setting.
• Strong understanding of relevant legislation, policies, and procedures.
• Excellent assessment, planning, and intervention skills.
• Ability to work effectively both independently and as part of a team.
• Strong communication, organizational, and problem-solving skills.
