Are you a dedicated and experienced social worker with a passion for supporting adults through critical transitions? We are looking for a qualified Adult Social Worker to join our Transitions Team in North London. This is a fantastic opportunity to make a real difference in the lives of adults with learning disabilities as they move through important stages of their lives.

Key Responsibilities:

• Conducting comprehensive assessments to identify the needs of adults with learning disabilities and their families.
• Developing and implementing personalized transition plans that support individuals moving from children's to adult services.
• Providing expert advice and support to individuals, families, and carers, ensuring their voices are heard and their choices respected.
• Coordinating with various agencies, healthcare professionals, and community resources to ensure seamless transitions.
• Managing a caseload of complex cases, maintaining accurate records, and adhering to legal and policy frameworks.
• Promoting independence, wellbeing, and community integration for adults with learning disabilities.
• Advocating for individuals' rights and entitlements, ensuring they receive the best possible care and support.

Requirements:

• Must be Social Work England qualified.
• Proven experience in adult social care, specifically with learning disabilities.
• Strong assessment and planning skills, with the ability to develop person-centered support plans.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
• In-depth knowledge of relevant legislation, policies, and best practices in adult social care.
• Commitment to promoting equality, diversity, and inclusion in service delivery.
