Sheldon Phillips is currently recruiting for an Adult’s Social Worker to join the Front Door & Urgent Community Team in Stockport. This role is ideal for an experienced social worker who is confident in urgent response work, multi-disciplinary collaboration, and promoting independence for adults in need of support.
Key Responsibilities:
- Work alongside health colleagues to reduce unnecessary hospital attendance and promote community-based care.
- Undertake same-day visits and urgent community responses as required.
- Complete Care Act and short-term assessments, focusing on reducing dependency and promoting independence within an individual’s home.
- Provide a social work perspective and expertise within a multi-disciplinary team.
- Ensure that all interventions are strengths-based and person-centred, supporting individuals to achieve the best possible outcomes.
Requirements:
- Social Work England (SWE) registration.
- Experience in urgent response, adult social care, and Care Act assessments.
- Strong knowledge of safeguarding, hospital avoidance, and multi-disciplinary working.
- Ability to work efficiently in a fast-paced team, responding to urgent cases.
- A clean driving licence and access to transport are required due to the office-based nature of the role.
