Key Responsibilities:

  • Completing Assessments: Conduct comprehensive assessments to determine the needs and preferences of service users and their families.

  • Home Visits: Conduct essential face-to-face visits in the community to assess living conditions, provide support, and ensure the well-being of service users.

  • Safeguarding: Initiate and lead Section 42 enquiries, ensuring the safety and protection of vulnerable individuals at all times.

  • Carers Assessments: Evaluate the needs of carers, providing support and resources to ensure their well-being and ability to continue providing care.

  • Covering Duty: Respond promptly to urgent situations, covering duty shifts as needed to provide immediate support and intervention.

  • Mental Capacity Assessments: Conduct assessments to determine the mental capacity of service users, ensuring their rights and autonomy are respected.

  • Multi-Disciplinary Meetings (MDM): Attend and actively participate in multi-disciplinary meetings, collaborating with other professionals to develop comprehensive care plans.

Requirements:

  • Ability to work efficiently in a fast-paced environment, managing multiple tasks and priorities effectively.

  • Competency in managing a caseload, demonstrating strong organizational and time-management skills.

  • Proficiency in working collaboratively within a multi-disciplinary team, fostering effective communication and cooperation.

  • Excellent working knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding protocols.

  • Familiarity with MOSAIC or similar social care software would be advantageous.

  • Registered with Social Work England

Brief on Key Tasks (3-6 months):

  • Complete comprehensive assessments for service users.

  • Conduct regular home visits to ensure the well-being of individuals and families.

  • Lead and manage Section 42 safeguarding enquiries.

  • Perform carers assessments, providing support and resources as needed.

  • Cover duty shifts to respond promptly to urgent situations.

  • Conduct mental capacity assessments in accordance with legal requirements.

  • Participate actively in multi-disciplinary meetings, contributing to holistic care planning.
