Havering | Locum | £41.10 per hour | Hybrid Working
Havering Council is seeking an experienced Advanced Practitioner to join their 16+ Team. This role focuses on supporting older adolescents and care leavers, ensuring they are prepared for independence and receive the right support to achieve positive outcomes into adulthood.
You will take a lead role in managing complex cases, driving best practice, and supporting less experienced colleagues within a specialist service working with young people aged 16 and above.
In this role, you will:
- Manage a complex caseload of young people aged 16+ including care leavers
- Support pathway planning and transitions into independence
- Undertake assessments and develop outcome-focused support plans
- Work closely with housing, education, and partner agencies
- Provide mentoring, coaching, and support to social workers within the team
- Lead on complex cases and provide expert guidance on risk management
- Ensure high-quality recording and compliance with statutory duties
What You’ll Need:
- Social Work England Registration
- Social Work Qualification
- Significant post-qualified experience in children’s social care
- Experience working with adolescents, 16+ or leaving care services
- Strong understanding of safeguarding, risk, and transition planning
- Ability to support practice development and mentor colleagues
- Excellent assessment, analytical, and decision-making skills
