Communities Social Worker - Adults Services - Ealing Council

Rate: £32 per hour
Location: Ealing
Working Arrangement: Hybrid

Ealing Council is offering an exciting opportunity for a Communities Social Worker to join their Adult Services Team. This role focuses on delivering person-centered support and promoting independence for adults within the community.

Responsibilities:

  • Conduct strengths-based assessments in line with Ealing’s Better Lives approach.

  • Develop tailored care and support plans, empowering adults to live independently.

  • Work collaboratively with families, carers, and external agencies to deliver effective interventions.

  • Undertake Safeguarding Adults investigations in accordance with the Care Act 2014.

  • Support adults with complex needs, including mental health, learning disabilities, and physical disabilities.

  • Maintain accurate case records and ensure compliance with statutory responsibilities.

  • Participate in regular supervision and contribute to service development.

Requirements:

  • Qualified Social Worker with Social Work England registration.

  • Experience in adult social care, including strengths-based assessments and safeguarding.

  • Strong knowledge of the Care Act 2014 and Mental Capacity Act.

  • Excellent communication and problem-solving skills.

  • Commitment to Ealing’s community-centered practice model.
