Communities Social Worker - Adults Services - Ealing Council
Rate: £32 per hour
Location: Ealing
Working Arrangement: Hybrid
Ealing Council is offering an exciting opportunity for a Communities Social Worker to join their Adult Services Team. This role focuses on delivering person-centered support and promoting independence for adults within the community.
Responsibilities:

Conduct strengths-based assessments in line with Ealing’s Better Lives approach.

Develop tailored care and support plans, empowering adults to live independently.

Work collaboratively with families, carers, and external agencies to deliver effective interventions.

Undertake Safeguarding Adults investigations in accordance with the Care Act 2014.

Support adults with complex needs, including mental health, learning disabilities, and physical disabilities.

Maintain accurate case records and ensure compliance with statutory responsibilities.

Participate in regular supervision and contribute to service development.
Requirements:

Qualified Social Worker with Social Work England registration.

Experience in adult social care, including strengths-based assessments and safeguarding.

Strong knowledge of the Care Act 2014 and Mental Capacity Act.

Excellent communication and problem-solving skills.
