Leaving Care Social Worker

Swindon Borough Council are looking for a Qualified Social Worker to join their Positive Futures Team. This is a Full-Time Locum Social Work role.

About The Team:

Within the leaving care service, you will manage a caseload of young people who are preparing to leave care. You will also be working with the allocated Pathway Advisors for the young person who are also based within our service.  The service links in early with the young people transferring for other services and you will lead on the completion of pathway plan assessments and the initial pathway plan

Key Responsibilities:

  • Lead on the completion of pathway plan assessments and the initial pathway plan.
  • It will be your ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor.
  • Managing your own caseload, you will also be required to be part of our duty system that provides additional support to our young people alongside our Pathway Advisors.

 

About You:

To apply for this role, you will need least 3 years post qual experience working with children and young adults. BA(Hons) in Social Work and to be registered with Social Work England (SWE).
