As a Qualified Social Worker in our Learning Disability Team, you will play a vital role in supporting young people and their families as they prepare for adulthood. You will work closely with Children’s Services, Education, and Health to ensure these individuals receive the right support from the right people.
The role will primarily focus on conducting assessments and support planning for high-cost placements and care packages, with a particular emphasis on young people approaching adulthood.
Key Responsibilities:
- Conduct Care Act 2014 compliant eligibility assessments for young people and their carers to identify eligible social care needs.
- Develop person-centred, strengths-based support plans, ensuring the outcomes reflect the young person’s needs.
- Complete comprehensive Continuing Healthcare (CHC) checklists and collaborate with NHS partners to secure CHC funding for eligible individuals.
- Monitor and review support plans, ensuring they meet changing needs while providing value for money.
- Safeguard and promote the welfare of young people, adhering to legislation and policy, including completing safeguarding investigations and work required by the Court of Protection.
- Manage a complex and varied caseload with the ability to make sound professional judgments under supervision and guidance.
- Work collaboratively with multidisciplinary teams, including Children’s Services, Education, healthcare professionals, and community partners, to take a holistic approach in meeting the needs of young people.
About You:
- A registered social worker with Social Work England.
- Experience in Adult Services, particularly working with individuals with learning disabilities or transitioning from Children’s to Adult Services.
- Strong understanding of the Care Act 2014, safeguarding processes, and CHC funding.
- Ability to assess risk, advocate for young people’s rights, and manage a complex caseload.
- Collaborative team player, with the ability to work effectively with a range of professionals.
