Are you a highly skilled and motivated professional with a passion for driving excellence in adult social care? We are seeking a dynamic Practice Manager to lead our Adults Community Team. In this pivotal role, you will oversee the delivery of high-quality services to adults in the community, ensuring their needs are met effectively and efficiently.

Key Responsibilities:

  • Reporting to a Team Manager, to manage and professionally supervise a number of qualified and support staff and to support other colleagues.
  • To be responsible for ensuring the delivery of high quality and legally compliant social care practice to agreed standards for the staff within the remit of the post.
  • Undertaking the full legal range of social care assessments and developing person-led support plans that meet the needs and outcomes of individuals.
  • Managing a complex caseload to ensure timely and effective intervention.
  • Maximizing the independence and wellbeing of adults and older people through proactive support and advocacy.
  • Being a member of one of the following teams: General Adults Complex Care.

Requirements:

  • Significant experience in adult social care, with a track record of leadership and management within a community setting.
  • Qualification in social work or a relevant discipline.
  • Registration with the appropriate regulatory body (e.g., Health and Care Professions Council).
  • Strong leadership and team management skills, with the ability to inspire and motivate others.
  • Excellent communication and interpersonal skills, with the ability to build effective working relationships with a range of stakeholders.
  • Sound understanding of relevant legislation, policies, and best practice guidelines in adult social care.
  • Commitment to promoting equality, diversity, and inclusion in service delivery.
