We are seeking an enthusiastic and skilled Qualified Adult Social Worker to join our vibrant and fast-paced Assessment and Intake Team in Central London. This community-based team is dedicated to providing initial assessment services for the Older Persons and Physical Disability Service. The role involves undertaking short-term pieces of work, typically up to 6 weeks, to ensure timely and effective support for individuals in need.

Key Responsibilities:

  • Conduct comprehensive Care Act Assessments to determine individuals' needs and establish long-term packages of care.
  • Perform urgent assessments for care and respite to address immediate needs.
  • Carry out carers assessments to support those providing care to loved ones.
  • Assess and facilitate placements in residential and nursing home settings.
  • Collaborate with nurse assessors to jointly undertake some Decision Support Tools (DSTs) assessments.
  • Complete Mental Capacity Assessments to evaluate individuals' ability to make informed decisions.
  • Manage the initial concern phase of adult safeguarding, before passing enquiries to the case management team.
  • Work efficiently in a dynamic environment to deliver high-quality, person-centered care and support.

Essential Requirements:

  • A recognised Social Work qualification and current registration with Social Work England.
  • Proven experience in adult social work, particularly within assessment and intake settings.
  • Strong understanding of the Care Act 2014 and Mental Capacity Act.
  • Ability to perform comprehensive assessments and develop effective care plans.
  • Excellent communication and interpersonal skills to work effectively with service users, families, and multidisciplinary teams.
  • Ability to manage a fast-paced workload and complete short-term pieces of work efficiently.

Desirable Skills:

  • Experience in older persons and physical disability services.
  • Familiarity with the processes and criteria for residential and nursing home placements.
  • Knowledge of safeguarding procedures and the ability to manage initial concerns.
