Are you a dedicated and experienced Social Worker with a passion for supporting individuals with complex needs? Join our Complex Needs Team in Central London, where you will play a vital role in providing tailored support and making a significant impact on the lives of those with complex requirements.

Key Responsibilities:

  • Caseload Management: Effectively manage a diverse caseload of individuals with complex needs, ensuring high-quality and person-centred support.
  • Holistic Assessments: Conduct thorough assessments and reviews in a holistic manner, focusing on the strengths and needs of individuals to create comprehensive support plans.
  • Support Planning: Develop and implement individualized care plans that promote independence, wellbeing, and improved quality of life.
  • Multi-Disciplinary Collaboration: Work collaboratively within a multi-disciplinary team, liaising with healthcare professionals, service providers, and other stakeholders to ensure integrated support.
  • Safeguarding: Identify and respond to safeguarding concerns, ensuring the safety and protection of vulnerable individuals.
  • Legislative Compliance: Ensure all work is carried out in compliance with relevant legislation, including the Care Act 2014 and the Mental Capacity Act.
  • Record Keeping: Maintain accurate, up-to-date records of all interventions and decisions.
  • Advocacy: Advocate on behalf of service users to ensure their voices are heard and their rights are upheld.

Requirements:

  • Must be registered with Social Work England.
  • Proven experience in working with individuals with complex needs.
  • Strong understanding of relevant legislation, including the Care Act 2014 and the Mental Capacity Act.
  • Excellent assessment, planning, and intervention skills.
  • Ability to work effectively within a multi-disciplinary team.
  • Commitment to promoting independence and improving quality of life for individuals with complex needs.
