We are seeking a highly qualified and experienced Adult Social Worker to join our dynamic Mental Health Team in South London. This is a unique opportunity to make a significant impact in the lives of individuals with complex mental health needs.
Key Responsibilities:
- Manage Complex Mental Health Cases: Provide expert management of complex adult mental health cases, ensuring the highest standards of care.
- Multi-disciplinary Collaboration: Work effectively with multi-disciplinary professionals to deliver comprehensive care.
- Facilitate Safe Discharges: Coordinate timely and safe discharges under Section 117, attending ward rounds as necessary.
- Liaison with Housing and Brokerage: Collaborate with housing services and brokerage to ensure appropriate support and accommodations.
- Court of Protection and Tribunals: Handle applications to the Court of Protection and represent cases in Mental Health Tribunals.
- Safeguarding Enquiries: Conduct thorough safeguarding enquiries to protect vulnerable adults.
- Travel Requirements: Must be a driver with own transport, able to travel within and outside the borough as required.
- Duty Rota Participation: Be an active part of the team duty rota, providing support and expertise as needed.
- Work Under Pressure: Demonstrate resilience and efficiency in a pressurised environment, prioritizing tasks effectively.
- Proactive Case Management: Exhibit a proactive approach to managing complex caseloads, ensuring timely and effective interventions.
Essential Qualifications and Experience:
- Registration: Must be registered with Social Work England.
- Experience: Proven experience in managing complex adult mental health cases.
- Multi-disciplinary Work: Demonstrated ability to work with multi-disciplinary professionals.
- Discharge Facilitation: Experience in facilitating safe discharges under Section 117.
- Court and Tribunal Work: Competence in handling Court of Protection applications and Mental Health Tribunals.
- Safeguarding: Experience in conducting safeguarding enquiries.
- Transport: Must have a valid driver's license and own transport for travel requirements.
- Team Participation: Willingness to participate in the team duty rota.
- Stress Management: Ability to work efficiently under pressure and manage priorities effectively.
- Proactive Management: Skilled in proactive management of complex caseloads.
