A local authority in Central London is seeking a dedicated and experienced Qualified Social Worker to join our Transfer of Care (TOC) Review Team, specializing in Adult Social Care for Older Adults and Physical Disabilities. This role involves providing a comprehensive, effective, and integrated social work service, ensuring the delivery of excellent social work practice within their practice framework. You will manage complex cases, assess complex needs, and work purposefully with families, support networks, and other professionals to manage risk, safeguard adults, and promote positive outcomes.

Key Responsibilities:

  • Deliver high-quality social work practice, assessing complex needs and planning interventions.
  • Work collaboratively with families, support networks, and professionals to manage risks and promote positive outcomes.
  • Conduct home visits to clients’ homes or nursing/residential placements to complete Care Act Assessments and Reviews for clients recently discharged from the hospital.
  • Provide a relationship-based social work service that responds timely within statutory guidelines.
  • Support team development through coaching and mentoring of students, newly qualified social workers, and less experienced practitioners.
  • Maintain 3-4 days per week office presence, with the flexibility of 1 day per week working from home for write-ups.

Requirements:

  • Qualified Social Worker registered with Social Work England.
  • Experience in adult social care, particularly with older adults and physical disabilities.
  • Strong ability to manage complex cases and conduct comprehensive assessments.
  • Excellent communication and collaboration skills.
  • Commitment to delivering relationship-based social work practice.
  • Ability to work both independently and as part of a team.
