We are seeking a dedicated and experienced Adults Mental Health Social Worker to join our dynamic team in North-West London. This role requires a committed professional who is passionate about providing high-quality mental health and social care services.

Key Responsibilities:

  • Care Coordination: Manage complex cases under the Care Programme Approach (CPA).
  • Care Planning: Develop co-produced care plans in collaboration with service users.
  • Risk Assessment & Management: Conduct comprehensive risk assessments and implement management strategies.
  • Safeguarding: Ensure the safety and protection of vulnerable adults through effective safeguarding practices.
  • Holistic Assessments: Perform thorough assessments of mental health and social care needs.
  • Care Act Assessments: Conduct and review assessments in line with the Care Act.
  • Carer Support: Work with carers and wider support networks to ensure comprehensive care.
  • Partnership Working: Collaborate with statutory and voluntary organisations to enhance service provision.
  • Liaison: Maintain strong communication with inpatient units and other relevant entities.
  • Record Keeping & Reporting: Ensure accurate and timely documentation and report writing.

Essential Requirements:

  • Experience: Proven experience working with adults with psychosis and complex needs.
  • Care Act Knowledge: Strong understanding and experience in conducting Care Act assessments and reviews.
  • Organisational Skills: Highly organised, with the ability to manage time effectively and prioritise workload.
  • Interpersonal Skills: Excellent communication skills to engage effectively with service users from diverse backgrounds.
  • Team Collaboration: Ability to work collaboratively within a multidisciplinary team of professionals.
  • Legislation Knowledge: In-depth knowledge of relevant legislation, including Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act.

Qualifications:

  • Registration: Must be registered with Social Work England.
