Adult Social Worker – 18+ Team
37 Hours per Week | Hybrid Working | Community-Based Role
Wokingham Borough Council is recruiting an experienced Adult Social Worker to join their busy and rewarding 18+ Team. This role focuses on supporting adults with a wide range of complex needs including learning disabilities, physical disabilities, long-term conditions, dementia, substance misuse, and homelessness.
The service supports residents across the adult lifespan – from transition into adulthood through to older age – aiming to promote independent living, support carers, and ensure individuals can live safely in their communities.
Key Responsibilities:
- Undertake strength-based assessments and direct work with adults aged 18+
- Carry out carers’ assessments and develop support plans
- Respond to safeguarding concerns under Section 42
- Work closely with multi-disciplinary professionals
- Support individuals with dementia, substance misuse, hoarding behaviours, and housing instability
- Promote independence and person-centred outcomes
- Manage a varied caseload efficiently and professionally
Working Arrangements:
- 37 hours per week
- Hybrid working – minimum 1 day in the office and regular community visits
- Based within the Adult Social Care service in Wokingham
- Free on-site parking and supportive team structure
Essential Requirements:
- Qualified Social Worker registered with Social Work England
- Minimum 3 years’ post-qualified experience in Adult Social Care
- Proven experience with safeguarding, community work, and complex case management
- Car driver with access to own vehicle
- Strong understanding of key legislation including the Care Act, MCA, and S42 safeguarding
Pay: Up to £38 per hour, depending on experience
