Job Description:
We are seeking an experienced and qualified Adults Social Worker to join our Hospital Team. The successful candidate will be responsible for delivering high-quality social work services, working closely with healthcare professionals, families, and other stakeholders to ensure safe and timely discharges from hospital settings.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals' needs, risks, and strengths within hospital settings.
  • Develop and implement tailored care plans, ensuring that all interventions are person-centered and promote independence.
  • Work collaboratively with multidisciplinary teams to facilitate safe and effective discharge planning.
  • Provide advocacy and support to adults, ensuring their voices are heard and their rights are upheld.
  • Manage a complex caseload, maintaining accurate and up-to-date records in accordance with statutory requirements.
  • Liaise with external agencies, including housing, healthcare, and community services, to coordinate ongoing care and support.
  • Ensure compliance with all relevant legislation, policies, and procedures, including the Care Act 2014 and safeguarding protocols.

Requirements:

  • Qualification: Must hold a Social Work Degree.
  • Registration: Must be registered with Social Work England.
  • Experience: A minimum of 2 years’ experience within adult social care, preferably within a hospital or similar setting.
  • Skills: Strong assessment and care planning skills, with the ability to manage complex cases and work effectively under pressure.
  • Knowledge: In-depth understanding of relevant legislation, policies, and best practices in adult social care.
  • Communication: Excellent interpersonal and communication skills, with the ability to build positive relationships with service users, families, and professionals.
