Are you a dedicated and experienced Adults Social Worker looking for a rewarding opportunity? Join our dynamic Hospital Team in Somerset, where you will play a vital role in supporting discharge planning and ensuring that patients receive the care they need as they transition from hospital to home or other care settings.

Key Responsibilities:

  • Collaborate with healthcare professionals to assess patient needs and develop effective discharge plans.
  • Conduct assessments and reviews, ensuring that care plans meet the individual needs of adults.
  • Liaise with patients, families, and external agencies to coordinate care and support services.
  • Ensure timely and safe discharge from hospital, reducing delays and promoting patient well-being.
  • Maintain accurate and up-to-date records in line with legal and procedural requirements.
  • Provide expert advice and support to patients and their families on social care options and resources.

Requirements:

  • Qualification: Degree in Social Work or equivalent.
  • Registration: Must be registered with Social Work England.
  • Experience: Proven experience in adult social care, preferably within a hospital setting.
  • Strong understanding of discharge planning processes and relevant legislation.
  • Excellent communication, assessment, and organizational skills.
  • Ability to work effectively within a multidisciplinary team.
