As part of our Initial Assessment Team, you will have the opportunity to make a real difference in the lives of adults new to social care.

Position Overview:

We are seeking a passionate and experienced Social Worker to join our Initial Assessment Team. You will work alongside a dedicated team of Occupational Therapists, Social Workers, and non-qualified staff, addressing a wide range of conditions and social situations. This role is pivotal in providing timely and effective support, promoting independence and well-being.

Key Responsibilities:

  • Conduct initial assessments for individuals new to social care services.
  • Apply the Care Act, utilizing a strengths-based approach and creativity to identify suitable solutions.
  • Promote a rehabilitation approach where appropriate, facilitating recovery and independence.
  • Organize long-term support plans for individuals with ongoing needs.
  • Work collaboratively with a multidisciplinary team to ensure comprehensive care.
  • Engage with diverse communities and address varied social situations.
  • Maintain up-to-date and accurate records in accordance with regulatory standards.

Requirements:

  • Social Work England Qualification: Essential
  • Proven experience in adult social care, particularly in initial assessment or front door services.
  • Strong knowledge of the Care Act and strengths-based approaches.
  • Excellent communication and interpersonal skills.
  • Ability to work creatively and collaboratively within a multidisciplinary team.
  • Commitment to promoting independence and person-centered care.
