We are looking for an experienced and motivated Qualified Social Worker to join our Older People and Disability Service within a central London local authority. This role offers a fantastic opportunity to work with older adults and individuals with disabilities, helping to improve their quality of life and ensuring they receive the support they need.
Key Responsibilities:
- Conduct assessments, care planning, and reviews to support older people and individuals with disabilities, ensuring their needs are met effectively and in line with statutory requirements.
- Work collaboratively with service users, their families, carers, and other professionals to develop comprehensive care plans.
- Undertake safeguarding investigations and ensure that any concerns are addressed promptly and in accordance with policies and procedures.
- Provide advice, guidance, and advocacy to individuals and their families.
- Maintain accurate and up-to-date records, ensuring all documentation meets legal and organisational standards.
- Work as part of a multidisciplinary team to deliver high-quality, person-centred services.
Essential Requirements:
- A recognised Social Work qualification (e.g., Degree or Master's in Social Work).
- Social Work England registration is mandatory.
- Proven experience working with older people and/or individuals with disabilities.
- Strong knowledge of relevant legislation, policies, and procedures, including the Care Act 2014.
- Excellent communication, assessment, and report-writing skills.
- Ability to manage a diverse caseload effectively and work under pressure.
