Are you an experienced and qualified Adults Social Worker looking for a role that offers flexibility and a chance to make a real difference? Our Local Authority client in Central London is seeking a Social Worker to join their team, focusing on support and safeguarding for adults. This role requires you to work on Fridays, Saturdays, Sundays, and Bank Holidays, making it perfect for those seeking a part-time opportunity with a high degree of autonomy and flexibility.
Key Responsibilities:
- Friday: Work on-site at the hospital, conducting assessments and facilitating safe discharges for adults in line with the Discharge to Assess model.
- Saturday & Sunday: Engage in primarily remote working, ensuring continuity of care and support for clients discharged over the weekend.
- Manage and support hospital discharges, collaborating with health professionals and other agencies to ensure timely and effective transitions from hospital to home or community settings.
- Carry out assessments, care planning, and reviews in line with Care Act responsibilities, ensuring that individuals receive appropriate support and safeguarding.
- Work closely with individuals, families, carers, and multi-disciplinary teams to create tailored care plans that promote well-being, independence, and safety.
- Maintain accurate and comprehensive case records in accordance with statutory requirements and local authority policies.
Essential Requirements:
- Social Work England Registration: Must be registered with Social Work England.
- Qualification: A recognised Social Work qualification (Degree/Diploma in Social Work or equivalent).
- Experience: Proven experience in hospital discharge planning in line with the Discharge to Assess model.
- Knowledge: Strong understanding of Care Act responsibilities and adult safeguarding principles.
- Skills: Excellent assessment, communication, and report-writing skills, with the ability to work both independently and as part of a multi-disciplinary team.
- Flexibility: Ability to work on Fridays at the hospital and remotely on weekends and Bank Holidays.
