We are seeking a dedicated and experienced Senior Social Worker to join our Preparing for Adulthood (PFA) Team within a vibrant and supportive local borough in Berkshire. This role offers the opportunity to make a significant impact on the lives of young adults transitioning from childhood to adult services, ensuring they receive the appropriate support and guidance to lead fulfilling and independent lives.
Key Responsibilities:
- Lead on complex case management, focusing on young adults with varying needs as they transition into adulthood.
- Conduct comprehensive assessments, develop personalized care plans, and facilitate multi-agency meetings to ensure a holistic approach to support.
- Provide expert advice and guidance to colleagues within the team and across the wider service, sharing best practices and contributing to continuous service improvement.
- Work collaboratively with other agencies, families, and young people to ensure that transitions are smooth, and that young people achieve their full potential.
- Maintain accurate and timely records in line with statutory requirements and local policies.
About You:
- Qualifications: Must be registered with Social Work England and hold a recognized social work qualification.
- Experience: Proven experience working within a Preparing for Adulthood or similar team, with strong expertise in handling complex cases involving young adults.
- Skills: Excellent communication, assessment, and report-writing skills. Ability to manage a varied caseload effectively and work under pressure.
- Location Flexibility: While the role requires 2-3 days in the office, we offer flexible working arrangements to support a healthy work-life balance.
