Are you a dedicated and compassionate Qualified Social Worker looking to make a difference in the lives of young people? Our client, the London Borough of Merton, is seeking talented individuals like you to join their dynamic 16+ Children in Care Team!
About the Role: As a Qualified Social Worker in the 16+ Children in Care Team, you will play a crucial role in supporting and empowering young people aged 16 and above who are in care. Your responsibilities will include conducting assessments, creating care plans, providing advocacy, and ensuring the well-being and safety of these vulnerable individuals. You will collaborate with multi-disciplinary teams, external agencies, and families to achieve positive outcomes for young people transitioning into adulthood.
Key Responsibilities:
- Conducting comprehensive assessments and reviews
- Developing and implementing care plans
- Providing direct support and guidance to young people
- Liaising with foster carers, schools, and healthcare professionals
- Advocating for the rights and best interests of young people
- Participating in team meetings and case conferences

Requirements:
- Qualified Social Worker with SWE registration
