Are you a dedicated and experienced Qualified Social Worker passionate about making a positive impact on the lives of young people? Our client, the London Borough of Southwark, is seeking a compassionate professional to join their 16+ Team. This is your chance to work collaboratively with looked after children and care leavers, providing the support they need to thrive.

Purpose of the Job: As a Qualified Social Worker in the 16+ Team, you will work in co-production with looked after children and care leavers, helping them cope with and achieve despite adverse childhood experiences. You will provide comprehensive assessments, planning, implementation, and evaluation of actions to support their welfare and development. This role requires a highly organized, trauma-informed practitioner with a can-do approach to ensure young people feel listened to, respected, and cared for.

Key Responsibilities:

  • Conducting thorough assessments of looked after children and care leavers
  • Developing and implementing personalized care plans
  • Providing ongoing support and intervention to young people
  • Collaborating with multi-disciplinary teams, external agencies, and families
  • Maintaining accurate and detailed case records
  • Advocating for the best interests and rights of young people
  • Participating in team meetings, case discussions, and ongoing professional development

Requirements:

  • Qualified Social Worker with current SWE registration
  • Experience working with looked-after children and care leavers
  • Strong assessment, planning, and organisational skills
  • Excellent communication and interpersonal abilities
  • Trauma-informed practice and a commitment to safeguarding and promoting the welfare of young people
  • Ability to work effectively within a multi-disciplinary team
