Our esteemed client, a reputable local authority in South East London, is seeking dedicated Qualified Social Workers to join their Safeguarding & Care Planning Service (East & West) within Children's Services. As a vital member of this team, you will play a pivotal role in safeguarding and promoting the welfare of children and young people in need.

About the Role and Team: The Safeguarding & Care Planning Service (East & West) works with children & families following transfer from the Referral & Assessment Service. Children in Need qualify if they meet through assessment Tier 2.5-3 on the Safeguarding Continuum of Need and Intervention. The service has a specialist Court Team but at times East and West practitioners will also hold care proceeding cases and work with children & families where the PLO framework has been initiated.

Responsibilities:

• Conducting assessments to identify the needs and risks faced by children and families.
• Developing and implementing care plans to address identified needs and promote positive outcomes.
• Providing direct support and intervention to children and families, including advocacy and practical assistance.
• Collaborating with multi-disciplinary teams and external agencies to coordinate services and support.
• Monitoring and reviewing the progress of cases to ensure effective intervention and positive outcomes.
