Our 18-25 Year Old Transitions Team is committed to ensuring smooth and effective transitions for young people, helping them achieve their full potential and independence.
Role Overview: We are seeking a qualified Social Worker to join our team on a temporary basis to undertake a backlog of Care Act Assessments and reviews. This is a hybrid role, requiring you to be in the office 2 days a week, with the flexibility to work remotely for the remainder of the week. Importantly, this position involves no case-holding responsibilities, allowing you to focus solely on completing assessments and reviews efficiently and effectively.
Key Responsibilities:
- Conduct Care Act Assessments and reviews for young adults aged 18-25.
- Ensure assessments are thorough, person-centered, and in compliance with legislative requirements.
- Work collaboratively with young people, their families, and other professionals to identify needs and develop appropriate support plans.
- Maintain accurate and up-to-date records of all assessments and reviews conducted.
- Utilize your expertise to contribute to the continuous improvement of our services.
Requirements:
- Must be a qualified Social Worker with relevant experience.
- Registration with Social Work England is essential.
- Strong knowledge and understanding of the Care Act and relevant legislation.
- Excellent assessment and analytical skills.
- Ability to work independently and manage your time effectively.
- Commitment to promoting the well-being and independence of young adults.
