Bath Local Authority is looking for an experienced and dedicated Qualified Social Worker to join our Adults Assessment Team, a short-term service supporting individuals post-hospital discharge and reablement through the Discharge to Assess model. This is a fantastic opportunity to make a significant impact within the community as part of our Social Care Assessment Service, consisting of a Senior Practitioner, Social Workers, and Social Care Practitioners dedicated to delivering exceptional care.

Role Overview:

  • Work Schedule: Monday to Friday, 9am-5pm (some flexibility offered), with no weekend or bank holiday working.

  • Primary Location: Based in Peasedown St John, Bath, but covering the Bath and North East Somerset region.

Key Responsibilities:

  • Manage a caseload of clients, carrying out Care Act assessments, Mental Capacity assessments, and providing person-centered care planning.

  • Oversee case management for adults, including leading s42 safeguarding enquiries.

  • Develop and implement care plans to support individuals in transitioning safely from hospital to community care.

Requirements:

  • A recognized Social Work qualification and registration with Social Work England.

  • Strong experience in adult social work, including knowledge of safeguarding and care planning.

  • Ability to conduct thorough assessments and manage complex cases effectively.
