As a Social Worker on our Adults Community Team, you will play a crucial role in supporting the well-being of adults and promoting independence. Key responsibilities include:

• Conducting assessments and developing tailored care plans for adults in the community.
• Collaborating with health professionals, families, and external agencies to ensure holistic support.
• Advocating for the best interests of adults and facilitating positive outcomes.
