As a Social Worker on our Older People with Disabilities Team, you will be responsible for assessing the needs of older adults with disabilities and coordinating appropriate support services. Key responsibilities include:

- Conducting comprehensive assessments to identify the care and support needs of older people with disabilities.

- Developing person-centered care plans in collaboration with service users, their families, and other professionals.

- Providing ongoing support and advocacy to ensure the well-being and quality of life of older adults with disabilities.
