Are you a dedicated and experienced Adults Social Worker looking to make a difference within a dynamic and supportive team? Our Local Authority client in Central London is seeking a skilled social worker to join their "Take Home and Settle" project team. This is an exciting opportunity to be part of an innovative project that supports adults transitioning back into their homes from hospital care, ensuring they have the necessary support and resources to settle back into their community.

Key Responsibilities:

  • Undertake comprehensive assessments of adults being discharged from hospital, ensuring their needs are identified and met in a timely manner.
  • Work closely with individuals, their families, carers, and other professionals to create tailored care plans that promote independence and well-being.
  • Provide advice, support, and interventions to facilitate a safe and effective transition from hospital to home.
  • Collaborate with multi-disciplinary teams, including health professionals and community services, to deliver holistic care and support.
  • Monitor and review care plans, making adjustments where necessary to ensure the best outcomes for service users.
  • Maintain accurate and up-to-date records in line with statutory requirements and local policies.

Requirements:

  • Social Work England Registration: Applicants must be registered with Social Work England.
  • Qualification: A recognised Social Work qualification (Degree/Diploma in Social Work or equivalent).
  • Experience: Proven experience working within adult social care, ideally in a hospital discharge or community setting.
  • Knowledge: Strong understanding of relevant legislation, policies, and procedures within adult social work.
  • Skills: Excellent assessment, communication, and interpersonal skills. Ability to work independently and as part of a multi-disciplinary team.
  • Commitment: A genuine commitment to improving outcomes for adults and promoting independence.
