Pay: Up to £32 ph

Contract: Full-time Locum

Bristol City Council is seeking dedicated and compassionate Qualified Social Workers to join our Discharge To Assessment Team, integrated within the discharge teams at Southmead Hospital and the Bristol Royal Infirmary. This is an exciting opportunity to make a meaningful impact on the lives of service users as they transition from hospital to home.

Key Responsibilities:

• Conducting Care Act assessments, mental capacity assessments, and other assessments in compliance with legislation.

• Developing comprehensive discharge plans that ensure service users receive appropriate support.

• Collaborating closely with health colleagues and medical staff to facilitate seamless patient care transitions.

• Enabling service users to leave the hospital in a timely manner with the necessary support in place.

Requirements:

• A recognized Social Work qualification.

• Registration with Social Work England.

• Ability to be present in the office every day, unless reasonable adjustments are needed.
