As a Senior Social Worker, you will be at the forefront of coordinating hospital discharges, ensuring individuals receive appropriate support. Key responsibilities include:

• Conducting thorough assessments to determine the needs of individuals undergoing hospital discharge.
• Collaborating with healthcare professionals, families, and community resources to facilitate seamless transitions.
• Providing expert guidance to the team, contributing to effective case management.
