We are a local authority in Central London committed to providing high-quality social care services. Our LD Transitions Team is dedicated to supporting adults with learning disabilities as they transition into adulthood, ensuring they have the resources, support, and advocacy they need to lead fulfilling lives.

Key Responsibilities

  • Case Management: Manage a caseload of adults with learning disabilities, ensuring that each individual receives personalized support and care planning.
  • Assessments and Reviews: Conduct comprehensive assessments and regular reviews to evaluate the needs and progress of service users.
  • Multi-Disciplinary Collaboration: Work closely with healthcare professionals, educators, families, and other stakeholders to create and implement effective support plans.
  • Advocacy: Advocate for the rights and needs of service users, ensuring they have access to appropriate services and opportunities.
  • Crisis Intervention: Provide timely and effective intervention in crisis situations, ensuring the safety and well-being of service users.
  • Documentation: Maintain accurate and up-to-date records of all interactions, assessments, and plans in accordance with legal and regulatory requirements.

Qualifications and Experience

  • Essential:
    • Qualified Social Worker (DipSW, CQSW, or equivalent)
    • Current registration with Social Work England
    • Experience working with adults with learning disabilities, particularly in transitions
    • Strong knowledge of relevant legislation, policies, and best practices in social work
  • Desirable:
    • Experience in a local authority or similar setting
    • Familiarity with hybrid working environments
