Qualified Social Worker – Older adults mental health team – North-East London

Role:  Qualified Social Worker  
Location:  North-East London
Team: Older adults mental health team.
Pay:  up to £34 per hour
Contract: Full-Time Locum
In the office: Hybrid

We are seeking an experienced and compassionate Qualified Social Worker to join our Older Adults Mental Health Team in North East London. This role involves working with individuals who have functional and organic (dementia) mental health diagnoses. As a care coordinator in a multi-disciplinary team, you will manage a caseload of around 20 cases, ensuring the delivery of high-quality care and support to older adults in our community.

Key Responsibilities:

  • Act as a care coordinator, managing a caseload of approximately 20 cases involving older adults with mental health diagnoses.
  • Complete and implement Care Act 2014 assessments and support plans tailored to individual needs.
  • Conduct Mental Capacity Assessments to determine individuals' ability to make informed decisions.
  • Lead safeguarding enquiries to protect vulnerable adults from abuse and neglect.
  • Work on Court of Protection cases to ensure legal compliance and the protection of individuals' rights.
  • Collaborate effectively with a multi-disciplinary team, including health professionals, to provide holistic care.
  • Develop and maintain positive working relationships with service users, their families, and carers, ensuring their voices are heard in the planning and delivery of care.

Essential Requirements:

  • A recognized Social Work qualification and current registration with Social Work England.
  • Significant experience in adult social work, particularly in mental health and working with older adults.
  • In-depth knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.
  • Strong assessment and care planning skills, with the ability to manage complex cases.
  • Excellent communication and interpersonal skills to work effectively within a multi-disciplinary team.
  • A commitment to promoting independence, dignity, and respect for older adults with mental health needs.

Desirable Skills:

  • Experience in conducting Court of Protection cases.
  • Knowledge of dementia care and best practices in supporting individuals with organic mental health conditions.
  • Ability to work under pressure and manage competing priorities.
