Team: Young Adults with Learning Disabilities (assessments and reviews of young adults between 18-25 years old)
Pay:  up to £32
Contract: Full-Time Locum

Role Overview: We are seeking an experienced Social Worker to join our team, focusing on assessments and reviews of young adults aged 18-25. The successful candidate will provide a high-quality social work service, addressing the needs of young adults with disabilities in accordance with legislation, regulations, and our council policies and procedures.

Key Responsibilities:

  • Manage a caseload of young adults with disabilities, ensuring timely and effective interventions.
  • Conduct comprehensive Care Act assessments and reviews, developing and implementing care plans that meet the individual needs of young adults.
  • Work collaboratively with other professionals, families, and carers to ensure a holistic approach to care and support.
  • Maintain accurate and up-to-date records, ensuring compliance with legal and procedural requirements.
  • Participate in team meetings, supervision, and continuous professional development activities.

Requirements:

  • Substantial experience as a Social Worker, particularly in care act assessments and reviewing.
  • Registration with Social Work England.
  • In-depth knowledge of the Care Act and relevant legislation.
  • Strong assessment, analytical, and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
