Qualified Social Worker – Adults SMART Service – London

Location: London Borough of Brent
Pay/Salary: Up to £33.67 per hour (Umbrella)
Office Working: Hybrid / Community Based

We are currently working with the London Borough of Brent to find an experienced Qualified Social Worker for their Adults SMART Service. This is a specialist opportunity working with adults experiencing complex and intersecting needs, including homelessness, mental health difficulties, drug and alcohol dependency and involvement with criminal justice or probation services.

The SMART Service works with adults who can often struggle to engage with traditional services and may face significant safeguarding risks and social exclusion. You'll take a flexible, person-centred approach to engaging individuals in the community, assessing their housing, daily living, health and social care needs and helping bridge the gaps between different agencies and services.

The team's interventions are underpinned by trauma-informed practice, assertive outreach, harm reduction and multi-agency working, with a focus on building engagement and achieving sustainable longer-term outcomes.

Duties

  • Carrying out comprehensive Care Act assessments within the community.
  • Managing work with adults experiencing multiple and complex needs, including homelessness, mental health, substance dependency and contact with criminal justice services.
  • Using assertive outreach and trauma-informed approaches to engage adults who may struggle to access traditional services.
  • Completing strengths-based assessments, reviews and person-centred support plans.
  • Identifying and responding to safeguarding concerns using a Making Safeguarding Personal approach.
  • Completing Mental Capacity Assessments and Best Interest work where required.
  • Working closely with Housing, Health, Mental Health, Substance Misuse, Probation and other partner services.
  • Developing creative and flexible interventions for individuals who have experienced repeated exclusion from services.
  • Supporting individuals to access appropriate accommodation, community resources and health and social care services.
  • Managing risk while promoting independence, choice and positive longer-term outcomes.
  • Maintaining high-quality, accurate and timely case records and reports.

What you need

  • Social Work Qualification.
  • Social Work England registration.
  • Enhanced DBS (provided by us).
  • Strong experience within statutory Adults' Social Care.
  • Experience completing Care Act assessments in the community.
  • Knowledge of the Care Act, Mental Capacity Act and relevant Adult Social Care legislation.
  • Experience working with adults with complex and multiple needs.
  • Strong safeguarding and risk assessment experience.
  • Experience of effective multi-disciplinary and multi-agency working.
  • Ability to work autonomously and creatively with individuals who may be difficult to engage.
  • A strong strengths-based and person-centred approach to practice.
