Somerset County Council is seeking a compassionate and experienced Qualified Social Worker to join our Children with Disabilities Team. In this rewarding role, you will work closely with children and young people with disabilities, ensuring they receive the support and services they need to thrive and achieve their potential. You will collaborate with families and other professionals to provide tailored care and interventions.

Key Responsibilities:

  • Case Management: Manage a caseload of children and young people with disabilities, ensuring their needs are met through effective assessment, planning, and review of care plans.
  • Assessments: Carry out detailed assessments of children’s needs in line with the Children Act 1989, focusing on promoting their welfare and safeguarding.
  • Care and Support Plans: Develop, implement, and regularly review care plans that are tailored to the specific needs of each child and their family.
  • Multi-Agency Working: Work in partnership with professionals from health, education, and other agencies to deliver coordinated services that address the holistic needs of the child.
  • Family Support: Provide advice, support, and advocacy to families, helping them navigate the services and resources available for children with disabilities.
  • Safeguarding: Ensure the safety and well-being of children by identifying any safeguarding concerns and taking appropriate actions.
  • Report Writing and Legal Duties: Prepare reports for reviews, case conferences, and court proceedings as required, and attend legal proceedings when necessary.
  • Record Keeping: Maintain accurate case records and documentation using Somerset County Council’s systems, ensuring all information is up to date.

Requirements:

  • Qualification: Degree in Social Work or equivalent, with registration in good standing with Social Work England.
  • Experience: Proven experience working with children with disabilities and their families, as well as a strong understanding of the challenges they face.
  • Skills: Excellent communication, assessment, and care planning skills, with the ability to build strong relationships with children, families, and professionals.
  • Knowledge: Strong understanding of relevant legislation, including the Children Act 1989, and the principles of safeguarding children with disabilities.
