Team: Leaving Care Team

Pay: Up to £46 ph

Contract: Full-time Locum

Blackburn Local Authority is seeking an experienced and dedicated Qualified Social Worker to join our Leaving Care Team. This role is an incredible opportunity to support young people as they transition from care to independent living, helping them build a strong foundation for their future.

Key Responsibilities:

• Provide tailored support to care leavers, empowering them to navigate the challenges of independent living.
• Develop and implement care plans focused on the needs, goals, and aspirations of each young person.
• Conduct assessments and reviews, offering guidance on housing, employment, education, and mental health resources.
• Collaborate with other professionals, including housing, health, and education services, to provide holistic support.

Requirements:

• A recognized Social Work qualification and registration with Social Work England.
• Proven experience working with young people in care or leaving care.
• Strong interpersonal skills and a commitment to advocating for the needs of young people.
