We are looking for an experienced and dedicated Senior Social Worker to join the Children with Disabilities Team at a local authority in East London. This role is vital in providing support to children with disabilities and their families, ensuring that their needs are met, and that they have access to the right services and care.
Key Responsibilities:
- Assessment and Care Planning: Conduct comprehensive assessments of children with disabilities and their families to identify their needs, strengths, and support requirements. Develop personalised care plans to ensure children receive appropriate services and support.
- Safeguarding: Maintain the safety and well-being of children with disabilities, recognising potential safeguarding issues and acting in accordance with safeguarding procedures.
- Multi-Agency Working: Collaborate with health professionals, education providers, and other external agencies to deliver a multi-disciplinary approach to supporting children and their families.
- Case Management: Manage a varied caseload of children with disabilities, ensuring timely intervention, support, and monitoring of care plans.
- Direct Support to Families: Offer practical support and guidance to families, helping them navigate services and providing emotional support as they care for their child.
- Service Development: Contribute to the continuous improvement of the Children with Disabilities service, identifying areas for enhancement and developing creative solutions to meet the needs of children and families.
- Team Collaboration: Work closely with other social workers and professionals in the team to ensure coordinated care and support. Participate in team meetings and case discussions.
- Legislation and Policy Compliance: Ensure that all work is carried out in accordance with relevant legislation, policies, and procedures, including the Children Act and safeguarding guidelines.
