Are you an experienced and dedicated social worker looking for a rewarding opportunity within a dynamic and supportive team? We are seeking a Senior Social Worker to join our Community Independence Wellbeing Team in West London. In this role, you will play a crucial part in promoting independence and enhancing the wellbeing of our residents.

Key Responsibilities:

  • Conduct comprehensive assessments of service users' needs and develop tailored care plans.
  • Provide expert advice, guidance, and support to individuals, families, and carers.
  • Collaborate with multidisciplinary teams to ensure a holistic approach to care.
  • Lead and manage complex casework, ensuring timely and effective interventions.
  • Support the development and implementation of policies and procedures to improve service delivery.
  • Mentor and supervise junior social workers and other team members.
  • Maintain accurate records and prepare detailed reports as required.
  • Ensure compliance with relevant legislation, policies, and standards.

Qualifications and Experience:

  • Must be registered with Social Work England (SWE).
  • Proven experience as a senior social worker within a community or similar setting.
  • Strong understanding of the principles of independence, wellbeing, and person-centred care.
  • Excellent assessment, planning, and case management skills.
  • Ability to work collaboratively with diverse stakeholders, including healthcare professionals, voluntary organizations, and service users.
  • Strong communication, organizational, and problem-solving abilities.
  • Commitment to continuous professional development and reflective practice.
