We are currently recruiting for an experienced Social Worker to join the Children with Disabilities Team at Herefordshire Council, offering a competitive rate of £33.30 per hour.

About the Authority

Herefordshire Council’s Children with Disabilities service is a specialist team made up of Social Workers and Family Support Workers who work closely with education, health, CAMHS, occupational therapy, physiotherapy, and respite providers to deliver coordinated support packages. The service places a strong focus on holistic child and family assessments, short breaks, safeguarding, and transition planning into adulthood.

Key Responsibilities

  • Hold a caseload of children and young people with disabilities requiring statutory social work involvement
  • Complete Child and Family Assessments, CIN plans, Child Protection work, and where needed CLA planning
  • Undertake direct work with children using communication methods tailored to their developmental needs
  • Coordinate specialist support packages including short breaks, respite, and family support services
  • Work in partnership with schools, SEND teams, health professionals, and carers
  • Support effective transition planning into adult services for young people approaching 18
  • Maintain clear, high-quality recording and outcome-focused care plans

Candidate Requirements

  • Qualified Social Worker with Social Work England registration
  • Experience within a Children with Disabilities / SEND / complex needs team
  • Strong safeguarding and assessment skills
  • Good understanding of child development, disability legislation, and transition work
  • Excellent multi-agency working and communication skills
  • Ability to build trusting, strengths-based relationships with families
