A renowned hospital in Sussex is seeking a Hospital Discharge Social Worker to join its team. This is an exciting chance to work for an organisation committed to providing the highest quality of care for its patients. The successful candidate will be responsible for liaising with multi disciplinary teams to ensure that all patients' discharge plans are managed efficiently and effectively.

They need a Qualified Social Worker that is ready to help vulnerable adults really succeed and affect change. There are lots of progression opportunities too, with regular panels in place for internal promotions. The Managers are there to help you achieve your goals as much as you are there to help them achieve theirs.

You will need to have at least completed an ASYE in adults services to be eligible for this Hospital Discharge post in Sussex acting as a Qualified Social Worker.

As well as an ASYE, you also need:

•    Social Work England Registration
•    Social Work Qualification
•    Right to Work in the UK
