Social Worker – Leaving Care (Independent Futures Team)


Location: London Borough of Islington
Rate: Up to £37 per hour
Start Date: ASAP
Hybrid Working & On-site Parking Available

We are looking for a passionate and dedicated Leaving Care Social Worker to join the Independent Futures Team. This is an excellent opportunity for a social worker with experience supporting young people transitioning out of care, helping them achieve independence and a stable future.

Role & Responsibilities:

  • Support young people aged 16+ who are transitioning from care into independent living.
  • Develop and implement personalized pathway plans that address each young person’s needs, including housing, education, employment, and emotional well-being.
  • Offer guidance on life skills, budgeting, and access to training or further education.
  • Collaborate with housing, education, health services, and other agencies to ensure a comprehensive support network for young people.
  • Conduct regular visits and maintain positive relationships with care leavers, ensuring their voices are heard and their aspirations are encouraged.
  • Ensure all records are maintained in line with statutory requirements, contributing to their overall development and independence.
  • Prepare reports, attend meetings, and advocate for young people’s best interests at all times.

Requirements:

  • Social Work England registration
  • 2+ years post-qualified experience, preferably working with care leavers or young people in a similar setting
