Key Responsibilities:
As part of the Children with Disabilities Team, your responsibilities will include:
- Assessment of Needs and Risk: Undertake detailed and holistic assessments for children with disabilities, identifying their specific needs, risks, and strengths.
- Care Planning: Develop and review care plans that focus on the individual needs of the child, ensuring the right support is in place for the child and their family.
- Support for Families: Work closely with families to identify additional services, including respite care and specialist education, to support the child’s development and well-being.
- Multi-Agency Collaboration: Work collaboratively with healthcare professionals, education providers, and other services to ensure a coordinated approach to care.
- Direct Work with Children: Engage in direct work with children to support their needs, using creative and child-centered approaches.
- Safeguarding and Risk Management: Take appropriate action to safeguard children, ensuring that all safeguarding concerns are addressed in line with statutory guidance and local policies.
- Review and Monitoring: Regularly review the child’s care plan to ensure it remains appropriate to their needs, providing ongoing assessments and adjustments as required.
- Prepare Reports: Write high-quality reports for child protection conferences, multi-disciplinary meetings, and other professional forums, providing evidence-based recommendations.
- Promote Positive Outcomes: Focus on achieving positive outcomes for children with disabilities, promoting their independence and social integration.
Requirements:
- Social Work England (SWE) registration is essential.
- Previous experience in working with children with disabilities or in a similar role.
- Strong knowledge of relevant legislation, including the Children Act 1989 and The Children and Families Act 2014.
- Excellent communication, assessment, and organisational skills.
