Social Worker – Transitions Team | Wakefield Council

Position: Social Worker
Team: Transitions (Adult Services)
Contract: Agency – 3 Months+
Hours: 37 hours per week
Pay Rate: £35 per hour (Umbrella)
Location: Grange View, Wakefield, WF1 2PW
Start Date: ASAP
Working Pattern: Monday–Friday, hybrid working

Sheldon Phillips are working with Wakefield Council to recruit two experienced Adult Social Workers to join their Transitions Team. This team supports young people with additional needs as they move from Children’s Services into Adult Social Care, ensuring a smooth, person-centred and well-coordinated transition into adulthood.

  • Complex casework involving transitions, learning disabilities, and preparing for adulthood

  • Regular supervision and structured workload

  • Two vacancies available with potential for contract extension
     

Duties and Responsibilities:

  • Manage a caseload of young people transitioning into Adult Services

  • Complete Care Act assessments and support planning

  • Work closely with families, education, health and SEND teams

  • Attend multidisciplinary meetings, safeguarding conferences and reviews

  • Promote independence and support young people’s goals around housing, education, health, and employment

  • Maintain accurate records in line with professional standards
     

Requirements:

  • Social Work qualification and SWE registration

  • Experience in Adult Services, ideally in a Transitions or Learning Disabilities team

  • Knowledge of the Care Act, Mental Capacity Act and safeguarding procedures

  • Strong communication and multi-agency working skills

  • Ability to manage complex cases with a proactive, strengths-based approach
