Are you a passionate, experienced Adults Social Worker looking for a role that’s all about community connection and meaningful outcomes? Slough Borough Council are looking for someone just like you to join their Adults Communities Team.

This is a hybrid role with a good balance between home-based working and time out in the community, supporting adults with a wide range of needs — including older people, individuals with physical disabilities, and those with mental health conditions.

You’ll be supporting residents to maintain their independence, access the right services, and remain at the heart of their communities.

🔍 What you’ll be doing:

  • Carrying out strengths-based assessments under the Care Act

  • Supporting with safeguarding enquiries and following through with protection planning

  • Working closely with families, carers, and community resources to build sustainable support networks

  • Developing and reviewing care and support plans tailored to the individual

  • Advising on and arranging care packages, Direct Payments, or referrals to specialist services

  • Championing person-centred approaches and promoting independence wherever possible

  • Liaising with multi-disciplinary professionals and voluntary sector partners

🙋 What we're looking for:

  • A qualified Social Worker registered with Social Work England

  • Previous experience in an adults community team (or similar setting)

  • Strong working knowledge of The Care Act, MCA, and safeguarding procedures

  • Great communication skills, a collaborative spirit, and a person-first mindset

  • Ability to manage a mixed caseload confidently and independently

  • Enhanced DBS on the Update Service (or willingness to complete one)
