Are you a dedicated Social Worker looking for an opportunity to join a dynamic team dedicated to supporting individuals with disabilities of all ages? We are seeking a compassionate and skilled professional to join our All Ages Disability Team.

Key Responsibilities:

• Conducting assessments and developing care plans for individuals with disabilities and their families.
• Providing advocacy, support, and guidance to promote independence and well-being.
• Collaborating with external agencies and professionals to coordinate comprehensive support services.
• Participating in multi-disciplinary team meetings and case reviews.

Requirements:

• Qualified Social Worker with registration with Social Work England.
• Experience working with individuals with disabilities across all age groups.
• Strong communication, assessment, and case management skills.
• Valid UK driver's license and access to a vehicle for work purposes.
